A Forma individual counseling appointment is $180 (55 minutes)
A Forma couples’ counseling appointment is $240 (85 minutes)
For comparison, the average cost for an individual counseling session in the Washington, DC area is currently around $235. Couples’ therapy is around $300 for 90 minutes. At Forma Counseling, we work to keep our rates affordable and lower the barrier for services.
Insurance
While we do not bill insurance companies directly, we can provide a statement of services for you to submit to your insurance company for reimbursement. Forma Counseling is considered an “out-of-network provider” by insurance companies.

Although Forma Counseling does not bill your insurer directly, that doesn’t mean your insurance doesn’t reimburse for service. Insurances generally cover a portion or all the costs of counseling.

Because each health insurance provider and plan has its own policies and coverage, here are some initial questions to ask your health insurance provider:
Does my plan cover out-of-network providers?
What is my yearly deductible and has it been met?
Are there limits for mental health coverage (e.g. number of sessions, length of sessions, type of diagnosis)?
What is the out-of-network reimbursement rate for the procedure code 90837 (individual 53+ minutes of psychotherapy)?
What is my co-payment for an out-of-network provider for a 90837 procedure code?

The procedure is typically straightforward. Forma Counseling will provide a client with a bill that includes all the information needed by an insurer. Most insurance companies allow you to submit this bill online. On average the insurer will send a reimbursement check within two to six weeks.

No Surprises Act / Good Faith Estimate
The No Surprises Act was enacted on January 1, 2022, and requires healthcare providers to offer a Good Faith Estimate (GFE) to patients at the beginning of treatment. Or, for current patients, from January 1, 2022, onward.
To help you get a sense of your potential Good Faith Estimate, keep in mind: in a typical year, with weekly therapy, one might have as many as 50 sessions. Thus, the annual Good Faith Estimate would be a range from your therapist’s fee x 1 to your therapist’s fee x 50.
